Attention Deficit Disorder Evaluation Scale - Fourth Edition (ADDES-4)

Purpose: To evaluate Attention-Deficit/Hyperactivity Disorder (ADHD) in children and youth.

Time: School Version: 15 minutes; Home Version: 12 minutes

The Attention Deficit Disorder Evaluation Scale 4th ed. (ADDES-4) enables educators, school and private psychologists, paediatricians, and other medical personnel to evaluate and diagnose Attention-Deficit/Hyperactivity Disorder (ADHD) in children and youth from input provided by primary observers of the student’s behaviour.

The scale is available in two versions:

1. School Version: A reporting form for educators
2. Home Version: A reporting form for parents
