Conners 3 Self-Report DSM-5 QuickScore Form (pkg 25) - Conners 3–SR

The Conners Rating Scale - Third Edition (Conners 3) offers a thorough assessment of ADHD in children and adolescents from 6-18 years of age. The Conners 3 also addresses comorbid disorders such as Oppositional Defiant Disorder and Conduct Disorder.

Ratings can be obtained from parents and teachers, and a self-report form is available. All forms are available in full-length and short-length versions.

The Conners 3 Long Forms (for Parents, Teachers and Self) have been updated to provide a new scoring option for the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition™ (DSM-5™) Symptom Scales.

The Conners 3 Self-Report DSM-5 QuickScore Form (Conners 3-SR) contains 99 items. It is completed by the child or adolescent about themselves, and assesses behaviours and other concerns in children from the age of 8-18.

The child/adolescent writes on the external layers of the form, and the results then transfer through to the hidden scoring grid within. The examiner uses these internal layers to calculate the results. All QuickScore Forms include norms conversion tables (to convert the raw scores).

The full-length version provides more comprehensive results, and is recommended for both initial evaluations and comprehensive re-evaluations.
