The Symbol Digit Modalities Test (SDMT) detects cognitive impairment, by quickly screening for organic cerebral dysfunction in both children and adults.
The patient substitutes a number, either orally or written, for randomised presentations of geometric figures - Using a reference key, the examinee has 90 seconds to pair specific numbers with given geometric figures.
The SDMT AutoScore Test Form is completed by the examinee. They are provided instructions by the examiner on how to complete the form, and mark their responses directly onto the back page.
The top sheet of the Test Form is later removed by the examiner, to reveal the carbonated scoring sheet, providing the total number of correct responses.

S1: Psychology
• Masters degree in Psychology, OR evidence of equivalent level of study.
S2: Speech Therapist
• Masters degree in Speech Therapy/Speech Pathology, OR evidence of equivalent level of study.
S3: Occupational Therapist
• Masters degree in Occupational Therapy, OR evidence of equivalent level of study.
