Caregiver-Teacher Report (C-TRF) 1.5-5 Forms (pkg 50)

The Child Behavior Checklist for Ages 1.5-5 (CBCL 1.5-5) obtains parents', day-care providers', and teachers' ratings of 99 problem items plus descriptions of problems, disabilities, respondent concerns about the child, and the best things about the child.

The *Caregiver-Teacher Report (C-TRF) 1.5-5 Forms * are completed by day-care providers, teachers and others who see a child in a group of at least four children. The C-TRF requests demographic information about the child, plus information about the role of the respondent (i.e. caregiver or teacher), how well they know the child, and the context in which they encounter the child.

The C-TRF shares problem items with the CBCL 1.5-5 (completed by parents), but substitutes group situation items for family situation items.

The respondent will rate problem items as 0 for not true, 1 for somewhat or sometimes true and 2 for very true or often true based on the preceding two months.

After all the items have been rated, there are open-ended items that ask the respondent to describe the child, and what they perceive are the best things about the child.
