Child Behavior Checklist (CBCL) 6-18 Forms (pkg 50)

The Child Behavior Checklist (CBCL) School Age 6-18 enables practitioners to efficiently obtain standardised ratings and descriptive details of children's functioning as seen by parents (using the CBCL 6-18) and teachers (using the TRF 6-18). The Youth Self Report (YSR) 11-18 Form also obtains the youth's view of their own functioning.

The Child Behavior Checklist (CBCL) 6-18 Forms are completed by parents, parent-surrogates and others who see the child in a family-like context. It requests demographic information about the child, asks respondents to indicate their name and relationship to the child (mother, father, foster parent etc.), and the respondents occupation (optional).

The respondent completes the competence items on the first two pages, followed by the open-ended items for describing the child's disabilities/illnesses, what concerns them about the child, and what they feel are the best things about the child.

Then, on pages 3 and 4, they complete their ratings for behavioural, emotional and social problems. Respondents rate each problem 0 for not true, 1 for somewhat or sometimes true and 2 for very true or often true based on the preceding six months.
